What do you mean by "for no reason"? What was the termination code?

Also, if you're still having trouble with this, can you additionally send
your inlist_shock_part5 (where the failure actually occurs)?

If it successfully ran through shock_part4, but crashed in shock_part5, it
is often (but not always) because of trouble rescaling the nickel profile.
How much Nickel are you using?

I just ran this with my own shock_part5. The termination code is useful.

                                       start_m > stop_m
 1.1537291901136893D+01    1.1488135937630283D+01
STOP extras_startup

This tells us that the mass coordinate of the shock has already exceeded
the mass coordinate that it is supposed to stop at.

Inspecting the shock_part4.mod file, it looks like there are supersonic
outward velocities at the surface (of a few x 10^5 cm/s) but the (few x
10^8 cm/s) shock is only out at ~150Rsun. I bet this is causing the
failure. The high positive velocities (u) appear in final.mod as well, so
it's possible this problem only emerges when the supernova shock gets far
enough that these surface effects are high enough Mach number for MESA to
consider them to be the "supernova shock" (defined as the zone with the
highest mach number in the model, I believe).
Getting rid of the surface velocities may be a problem that out-of-the-box
MESA cannot solve on its own, but you could try with stellar engineering as
we've discussed in earlier threads.
Another suggestion is that you could also modify the routine setting
start_m and stop_m in extras_startup run_star_extras to provide a different
definition of the mass coordinate of the shock which does not mistake the
(low but still supersonic) surface velocities as the supernova shock.
